Express middleware: captures real traffic, redacts sensitive fields, hands samples to @vayo-hq/schema-engine. The only Vayo package allowed to import Express types.

MongoDB implementation of VayoDbAdapter (native driver, not Mongoose) plus vayo init migrations. Implements the BYODB layer.
